Clarity PSO - A Patient Safety Organization

The Patient Safety Structural Measure is an attestation-based measure that assesses whether hospitals have a structure and a culture in place that prioritizes safety. As part of the measure hospitals must attest to meeting the five domains below.

Hospitals must be able to attest to all statements within each domain in order to receive a point for that domain. With each domain counting for one (1) point, hospitals affirming all statements will receive the maximum of 5 points. CMS proposes that it will begin publicly reporting a hospital’s performance score, from zero (0) to five (5) points, on Care Compare starting in fall 2026.

Domain 1: Leadership Commitment to Eliminating Preventable Harm

Domain 2: Strategic Planning & Organizational Policy

Domain 3: Culture of Safety & Learning Health System

Domain 4: Accountability & Transparency

Domain 5: Patient & Family Engagement
